8-Step Problem-Solving Template

Problem Statement:

Use this template to work through a problem, using the 8-step problem-solving model.

1. Identify the Problem (What's Going On?)

  • Describe the problem clearly and concisely:

    • What is happening?

    • Where is it happening?

    • When is it happening?

    • Who is involved?

  • Problem Statement:

2. Gather Data (What Do We Know?)

  • List all the information, facts, and data related to the problem.

  • What evidence do you have to support the problem statement?

3. Analyze Data (What Are the Root Causes?)

  • Analyze the data to determine the underlying causes of the problem.

  • Use tools like:

    • Five Whys

    • Fishbone Diagram (Ishikawa)

    • Pareto Chart

4. Generate Solutions (What Could We Do?)

  • Brainstorm a list of potential solutions to address the root causes.

  • Consider all possible options, even if they seem unconventional at first.

5. Select Solution (What's the Best Thing to Do?)

  • Evaluate each potential solution based on factors like:

    • Feasibility

    • Effectiveness

    • Cost

    • Time

    • Impact

  • Choose the best solution or combination of solutions.

6. Plan Implementation (How Do We Go About It?)

  • Develop a detailed plan for implementing the chosen solution.

  • Include:

    • Specific steps

    • Timeline

    • Resources needed

    • Responsibilities

    • Metrics for success

7. Implement and Test (Have We Solved the Problem?)

  • Put the implementation plan into action.

  • Monitor the results and collect data to assess whether the solution is working.

  • Evaluate the overall effectiveness of the problem-solving process and the solution.

  • Identify any lessons learned.
