8-Step Problem-Solving Template
Problem Statement:
Use this template to work through a problem, using the 8-step problem-solving model.
1. Identify the Problem (What's Going On?)
Describe the problem clearly and concisely:
What is happening?
Where is it happening?
When is it happening?
Who is involved?

2. Gather Data (What Do We Know?)
List all the information, facts, and data related to the problem.
What evidence do you have to support the problem statement?

3. Analyze Data (What Are the Root Causes?)
Analyze the data to determine the underlying causes of the problem.

Fishbone Diagram (Ishikawa)
Pareto Chart
Root Causes Identified:
4. Generate Solutions (What Could We Do?)
Brainstorm a list of potential solutions to address the root causes.
Consider all possible options, even if they seem unconventional at first.
Possible Solutions:
5. Select Solution (What's the Best Thing to Do?)
Evaluate each potential solution based on factors like:
Feasibility
Effectiveness
Cost
Time
Impact
Choose the best solution or combination of solutions.
Chosen Solution:
6. Plan Implementation (How Do We Go About It?)
Develop a detailed plan for implementing the chosen solution.
Include:
Specific steps
Timeline
Resources needed
Responsibilities
Metrics for success
Implementation Plan:
7. Implement and Test (Have We Solved the Problem?)
Put the implementation plan into action.
Monitor the results and collect data to assess whether the solution is working.

8. Continue to Improve (Can We Improve on What We Have Done?)
Evaluate the overall effectiveness of the problem-solving process and the solution.
Identify any lessons learned.
Determine if the solution can be further improved or standardized.
Plan for ongoing monitoring and continuous improvement.
